Dr. Boller is not in-network with any insurance companies (meaning she does not accept insurance). Fees are self-pay only. Payments accepted include cash, check
and all major credit cards. An invoice for your use for out-of-network insurance benefits will be provided upon request.
Effective January 1, 2022, the No Surprises Act, which Congress passed as part of the Consolidated Appropriations Act of 2021, is designed to protect patients from
surprise bills for emergency services at out-of-network facilities or for out-of-network providers at in-network facilities, holding them liable only for in-network cost-sharing amounts. The No Surprises Act also enables uninsured patients to receive a good faith estimate of the cost of care. Review this document for an explanation of your rights and protections under this Act.

Step 1: Therapy Assessment
Individual – 90 min session
Before individual therapy begins, forms will be provided for you to complete and return to Dr. Boller. Once completed forms are received, your Therapy Assessment will be scheduled. Our first session is a 90 minute assessment session. This is usually scheduled via telehealth for ease of scheduling, but also could be in person. The purpose of this session is to understand your goals for therapy, understand your main concerns, and create a treatment plan. This plan will be the blueprint for our therapy together. This session is $300.00.

Step 2: Therapy
Individual – 50 minute session
After you complete your assessment, therapy will begin. Sessions are currently being scheduled for in person or telehealth. Therapy is usually initially scheduled every week at the same day/time. Therapy sessions are 50 minutes. Each session is $200.00
